Privacy Policy

What information we collect

When you register as a patient, we collect your personal and health information that helps us provide you with safe and effective care. This may include:

  • Your name, date of birth, address, and contact details e.g. Phone and email address. 
  • Relevant health information, medical history, and treatment notes. 

How we collect your information

You may provide information by: 

  • Filling in forms online or in person
  • Giving us details over the phone or in consultation. 

If you complete a paper form, we will scan and upload this informaiton securely onto our GDPR-compliant, encrypted practice management system (Cliniko), if required the original paper copy can be disposed of using appropriate channels. 


How we use your information

We use your information to 

  • Provide you with healthcare services
  • Keep accurate medical records
  • Contact you about your appointments and care. 

We will not use your personal data for marketing unless you give us explicit consent to do so. 


How your information is stored

Your information is kept confidential. We will only share it with: 

  • Other heathcare professionals involved in your care, with your knowledge and consent, unless required in an emergency. 
  • Third parties where we are legally obliged to do so. This may include:

  1. Safeguarding
  2. Public Health Concern 
  3. Court orders
  4. Crime and security 

How long we keep your information

Your data will be retained no longer than is absolutely necessary, following the NHS records management code of practice "standard" retention periods. 


Your rights

You have the right under the Data Protection Act 2018 to request access to view or obtain a copy of the information we hold about you, and to request for this to be modified if inaccurate. 

  • The right to erasure - to request that your personal data is removed from our system. Under some circumstances, we have a legal obligation to maintain a copy of your data for statutory reasons. 
  • The right to restrict processing - to limit how we use your data. 
  • The right to data portability - to reiceve your data in a portable format. 
  • The right to object - to certain types of processing e.g., marketing 
  • Automated processing - the right to object to the way we automatically process your data. We do not currently carry out automated processing of your data. 

If you are not satisfied with how we handle your data, you have the right to complain to the information Commissioner's office (ICO).
